Hm. Is this an internal or external modem?

If internal, is it PCI or ISA or ?

If pci, could you
# lspci and report back what the modem claims to be?

also, could you attach a copy of the output of dmesg?
# dmesg > /tmp/dmesg.txt
attach file "/tmp/dmesg.txt"
